Please bring the following documents to orientation if you have not already turned those in:

Social Security Card
Proof of Residency (Utility bill: power, water, gas, cable)
Immunization Certificate (Form 3231)
Eye, Ear, Nutrition, and Dental (Form 3300)
If you receive Food Stamps, SSI, Medicaid, TANF, or PeachCare, please bring verification that you receive assistance.
One parent or guardian is REQUIRED to attend orientation. You do not have to bring your child to orientation. You will bring your child to Open House on Tuesday, August 5, from 3:00 to 5:00 to meet their teacher.
