For example, if you need a PowerSchool access code, completing the form ahead of time may allow us to have the needed document ready. A parent or guardian will simply need to show a valid photo ID for verification, and once verified, you may be able to pick up your information and be on your way.

The form is not required to receive help on Friday. If you have PowerSchool questions or need assistance, please stop by — we are here to help!

A parent/guardian must complete and sign the form before you can pick up your Chromebook. If you do not pick up your Chromebook today or tomorrow, you will have to wait until school starts to have one assigned to you. Don't miss your chance to get it before the first day of school!

📄 Chromebook Agreement Forms and Insurance Forms are available outside the front doors of CHS.
✍️ A parent/guardian must sign the Chromebook Agreement Form before a Chromebook can be issued.
🛡️ We highly encourage purchasing the $10 Chromebook Insurance, it's much less expensive than paying for repairs or replacement if damage occurs.

PRESS RELEASE: Polk School District is thrilled to announce that 55 students have been recognized by the College Board as AP Scholars based on their performance on the spring 2026 Advanced Placement (AP) exams! AP courses allow students to experience college-level curriculum in high school, with the dual purpose of preparing them for college-level work and of earning college credit. Exams are scored on a 1-5 scale, with a score of 3 or higher generally considered the level required for college credit.

This group represents the largest number of AP Scholars ever recognized in the Polk School District and demonstrates our continued commitment to providing the highest-quality educational experiences possible for all students. This winter, both Cedartown High School and Rockmart High School were also recognized by the Georgia Department of Education as AP Honor Schools.

There are three levels of AP Scholar awards; the requirements for each level and the list of students who achieve these levels are below.

AP Scholars: This honor is granted to students who receive scores of 3 or higher on three or more AP Exams.
