Pre-K & Kindergarten Registration

REGISTRATION HAS BEEN POSTPONED (See news article)  

 

Registration for pre-k and kindergarten students for the 2020-2021 school year is quickly approaching. We wanted to share with you the dates and times, as well as, what information will be needed at the time of registration. 

Reminder: All current pre-k students will still need to register for kindergarten. 

 

To register for kindergarten, your child must be five (5) on or before September 1, 2020. 

To register for pre-k, your child must be four (4) on or before September 1, 2020. 

 

Cedartown Registration

Dates: March 17-19, 2020 

Time:  8:00 a.m. to 12:00 p.m. and 1:30 p.m. to 4:00 p.m. each day

Location: Polk School District Board of Education

 

Rockmart Registration

Dates: March 31, 2020 - April 2, 2020

Time: 8:00 a.m. to 12:00 p.m. and 1:30 p.m. to 4:00 p.m. each day

Location: Nathan Dean Community Center

 

What do you need to bring with you? 

For your child to be registered, state law requires the following information:

  1. Certificate of Live Birth (certified copy only, NOT the hospital copy or copy of live birth)
  1. Immunization CertificateForm 3231 (obtain from the Health Department or Pediatrician) or appointment card.
  1. Hearing, Vision, Nutrition & Dental Certificate; Form 3300 (obtain at the Health Department or Pediatrician & Dentist) or appointment card.

 

Kindergarten students must have a new Hearing, Vision, & Dental Certificate for Kindergarten Registration.

           

The school will also need:

            4.  Copy of Child’s Social Security Card (this will be your child’s school ID number)

            5.  Utility Bill (power, gas, or water to show proof of residency)

 

Please bring your child with you to the registration for a brief communication and readiness screening.
